REGISTRATION

Registration Process:

Participants must register using the printable PDF application form. (Application Form). (Registration cannot be completed online.)

Once the registration application is submitted and accepted, enrollees will be sent course confirmation to the email address provided on the registration application. If the course is filled or if the information entered on the registration application is not complete, acceptance will be denied and the registrant notified.

Financial Support

Hotel accommodation based on double occupancy will be arranged by Smith & Nephew (S&N) at the Radisson Hotel, Kingston Harbourfront. All meals associated with the course will be provided by S&N. All other meals will be the responsibility of the course participant.

If the registrant is driving to the course, parking will be provided at the hotel.
If driving to the course is not an option, arrangements can be made for air or train travel.

Please advise on your registration application the reason why driving is not an option
and a S&N organizer will be in touch to discuss.

For any registrants requiring air or train travel, a reasonable fare will be provided by
S&N, as well as ground transportation. All travel will be co-ordinated through S&N.
